saving perfection for another day

we are married to our job. period.
it was a normal working days as any other. i was busy in the ward as usual.
I stood over to take ABG for an elderly patient at the acute cubicle. Sensed a flow from my nostril. Perhaps i caught a cold i thought, carelessly wipe away with my forearm (avoiding contact withthe latex gloves).
There is faint trail of blood stained on my forearm now. My nose were bleeding.
nothing to be alarmed about. i reacted coldly and calmly. as if it doesnt concerned me. Held my mind off it, continue taking the ABG.
my emotion bleached by the constant repeatative tasks.


finding myself not able to sleep again tonite despite weary from a lengthy day of work. my mind wandering off somewhere else, seeking comfort.
i’d usually take effort to do thing till perfection, but lately, i’d save perfection for another day.

a rapid transition from a “goyang-kaki” dude to a fast-moving life in orthopaedic department
in my department, we mostly handles cases with various kinds of fractures, cellulitis, diabetic foot, necrotising fascitis etc
my work revolves with the wards, the public clinic, the OT (operation theater), HDW calls and 1st-call
(ps: HDW = high dependancy wards)
works 7 days a week
if u wondering how i am doing? i am doing fine.
i think some frens got it worse getting posted in other departments
love my job, hates the long hours
pray for me and fellow collegues who are hard-at-work to saves lives every single day

Blogging the blog & talking the talk :)

Blogging in not a easy task, i must admit. Sometimes, there is nothing interesting to write about. Sometimes, u got so much to tell, but, u will be too occupied to sit down properly to blog.
I’ve left my blog un-updated for quite some time, yet, I felt not right to just post sketchy bits of stories. Today, i’m determined to pick up the phase and start writing more blogs. So, where do i start?
Lastweek, while i was unpacking my stuff from Moscow, i decided to clean up some of my old stuff too. Running through boxes after boxes of “treasure”, i carefully sort out all the things. Then, i found some photo albums from nyears ago. So i decided some photos should be scanned and be placed into the bog.

St.John
Here’s a photo during the school open day. I hadn’t joined st.john ambulance at that time. My senior asked me to helped out acting as casualty of a cardiac arrest victim :D

well, the truth is, we always keep it proffesional when it comes to St.Joh work. All drill are chronologically done to perfection. 15 external chest compression must be counted out loud every single tijme. (note: latest updated external chect compression ratio is 1:30).
Being part of St.john is one of the best memories back then. Great friends who goes through thick and thins.


Form 6
6 form is supposed to be all work and no play for most. But, somehow, for this SMART-ians bunch, it never lacks of laughter, warmth and drama :p

During the 2 years in 6 forms, we are actually very active in co-curriculum. Being in bio-class almost make us instant st.johner. We did lots of duties on site, highway patrol, and even joined forces to organise the Malaysia Book of Records for the largest Blook donation drive :)

I’m also involved in a Science project, which brought our team to national competition. Haha, the project won me a 3 minutes fame to be featured in tv3 and had my face printed on Popular Science magazine :p

We also joined choir and battle all the way to National level. Though we failed to win the first time, our school redeemed our lost by winning 3rd place the next year :D

Don't mind the ugly custome... we are one heart and united.

So much for Merdeka…

disclaimer: this article are not meant to be a subject for political discussion. All record of incident is true by a first person experience point of view.
I just came back from a gathering with some of my old friends from high schools 30 mins ago. It happens that some of us are in town (Kuantan), because it is Merdeka day, so my friends have day-off. 4 of us headed to “Santai” cafe to have a drink and chit chat.
At 12 midnite, a bunch of motorcyclist (some 35 of them) start appearing out of everywhere…most of them don’t wear helmets and a few jokers are making noises with their motorcycle. They all occupied the small crossroad just next to the “Santai” cafe. It appear as if they were waiting for their gang to parade/illegally road-race. It is rather annoying, and they attracts a lot of attention from the ppl inside the cafe.
Then it happens!
A young man in brown jacket ran down from the motorcycle, reaches for the handbag of a girl sitting 1 table behind mine, then sped-off with his friend on a bike. The whole thing happens while my back is facing them. So, the moment when the girl scream for help, i could only catch a glimpse of the guy already getting on the motorcycle ready to go.
They really caught us off-guard!
We were all really stunt by the act of crime, and quite frankly speaking, I have no idea how to react. I already stood up when the girl scream, but I could only watch them running away. I felt so useless, and helpless. It is a shitty feeling.
The worse part of the story is, the bunch of motorist still lurks around the junction for another 15-20 mins not wanting to leave. My friends (all 3 of them are girls) became very worried, as anything could just happen now. We dont know if another of them will come and try to rob us, or if they would attack ppl in groups.
Gees, so much for Merdeka.
I felt so sad for being a Malaysian. Watching a crime occur on the day our country suppose to celebrate its independence day. Why if we were indepedent, that we still need to be fearful of such things? maybe i am naive to ask, but is it too much to ask for a peaceful country?
Seriously, I curse those ppl!
sometimes I wonder, is it because our system had fail us? this is the product of our nations????

surviving ? :p

i’ve graduated for almost 2 month now… back in Malaysia for almost 1 month. Initially, i was expecting to start working by mid sept. but by the way things are heading now, i think i will only start work by oct.
so, wat do i do with my free time? i wonder :p
well, the whole aug month doesnt past without some surprises. Lots of things happened…
- gone to register so i could start work
- met my moscow frends for karaoke,
- gone to watch fireworks at putrajaya,
- gone to visit relatives and had a wonder meal with my cousins
- tag along for tour guiding a bit for some Russian friends who came over to malaysia for holiday :)
at other times, i spent them with my family (esp with nephews). I dont have any photos to be uploaded now, coz i am ……………………..L A Z Y lorrr
well, regarding how to spend the free time till the starting of my housemanship? the basic plan is to get prepared for it, so i wont be caught off-guard with arusty knowledge.
so now, i will prepare some card-notes (like those we use in debate :p) with the some fingertips stuff essential to survive.
Janet gave me a few short notes written by some HO. It is some fun reading, but i think it is about time i dug up some of my books and start picking up the phase.
Of course, all talk and no work might not be enough, but seriously, i kindda dun feel like going to do practicals at hospital while waiting for my housemanship to begin. Coz, i will definately be swarm with the same thing when i start.
well, let’s take one thing at a time. Perhaps the path will show-up sooner or later ;)

St.Petersburg Trip

09:35pm My Train to St.Petersburg is somewhere around 1am past midnite. Haven’t pack anything yet, was chatting with Soo Ann and she shooed me away so I could start packing. Packing wasnt really too hard, I got everything i need in just 20 mins. Then I send the email to my sis regarding the documents i needed to report-in with JPA once i’m back in M’sia.




11.02pm I started off late as usual :p rusehd to the metro being unsure what time the line will close down. In the wago, I double-checked the train ticket. It says 1:50am. That translate to 2 hours of boring transition at the train-station.
12:15am Found a nice spot to actually sit down in the Vokzal (Vakzal = train station in Russian). Thanks to Jeya and Sashi, I now have a map and a guidebook with me.Took the time to research and decided places to visit in St.Pete. Here’s what I’ve listed:
(1) St.Isaac Cathedral – must go up and see the view
(2) The admiralty – can’t locate it on the map. (map is in Russian)
(3) Winter Palace/Hermitage – open 1030-1700 (My friends told me that i should probably skip the Museum part because it could takes hours. But after reading that Hermitage is one of the world famous Museum (World No.8), i decided to go ahead with it. If I walked fast enough, i could just finish it up in 3 hours. I’m kindda looking forward to it, as I have been to Florance’s Uffize (World No.5) , London’s British Museum (World No.3) , Vatican Museum (World No.2)… I would see how it compares.
(4) Summer Garden & Summer Palace
(5) Christ of spilled blood Cathedral (it look like St.Basil in Moscow, but Jeya told me it look nicer)
(6) Paul & Peter Fotress?
(7) Peterhoff (For 2nd Day)
01:20am Boarded the Train. Found my seat and went off sleeping away just after the routine ticket check.
Day 01
10++ am, reached St.Pete. Had no trouble finding my way to the nearest metro station and headed for the hostel. The hostel is located strategically at Nevsky Prospecky, just behind the Kazans Cathedral. As i cross the street, I could see the cathedral of Christ of Spilled blood just in a distance.

Met a Frech girl when I got to the Cuba-Hostel, had a bit of chit-chat while waiting for the registration to check me in. After check-in, I refreshed myself with a shower and headed out to explore st.pete.
1st stop, I went to the Cathedral of Christ of Spilled Blood.Took some photos.

Then, i ventured toward the St.Isaac. Queued for the ticket booth for 5 mins, but the lady denied my of any ticket coz she didn’t have change for 500rub!!! I crossed the street and try to buy a bottle of water to get some change, but end up having to buy 2 bottle of Gastrol and 1 bottle of bonviva (coz the vendor apparently doesnt have enough change for me as well, tough luck!!)

Back to the ticket booth, they are selling tickets to the Collumnade and ticket to take photos seperately. However, it doesnt cost me much as I have a student pass.
Then, off i went to climb all the way to the collumnade. I think there is some 200 steps in total. Luckily, I came here 1st thing of the day. If i would have came after i walk all over the town, I don’t think i could make it up wihout panting.

The view from the collumnade is spectacular. I snaps lots of photos, but not many with me in it. (that’s the pain of travelling alone)
See the black plastic bag on my left hand? That will be the 3 bottle of water i bought to get enough change for the tickets!

Next stop, the Admiralty and Winter Palace (Hermitage). Nothing much to see at the Admiralty. It is a shipyard fort. Right in front of the Admiralty is the Hermitage. I went in and queued for the ticket. Though the queue was kindda long, I got in after some 15mins wait. The Museum is really beautiful. I am impressed with the interior of the building, and its collections of course.

Somewhere about 5pm, I left Heritage and go roaming around. Reached the Nevsky River. It happened to be the national Navy day :) Lots of Army ship and stuff



Day 02
Today’s plan is simple, PETEHOFF! :D Peterhoff is a palace.









About 4pm, left Peterhoff. Went to Summer Park. Then roam somemore.
8pm I walked back to the hostel. Still wondering how I’d spend the last few hours. Instead, I found myself chatting away at the lounge area of Cuba-hostel. Met Tim from Holland, 2 girls from Australia, and another dude from Canada. I’m quite surprise that the staff are actually being quite tolerant. I was supposed to be out of the place, but they let me hang-around, use the internet for free. Heck! they even serve me free teas.
12:50am I boarded the train without much trouble. But, okay, I’m now officially a magnet for old ladies. not only that I’m seated with one on my way to St.Pete, now I will ride back with one too. (Tough luck :p)

chances

we were assigned to intensive care unit today. We’re supposed to clerk all patients and do write-up on the managements. after a short introduction with our first patient, the conversation was halted, as the patient claimed that he doesn't feel fit to continue the talk. So, i told him i’ll get back to him afterwards so he would rest a bit more.
We proceed to clerk another patient and the nurse alerted us that someone in the ICU just crashed. So we rushed over and find the male patient we just talked to 2 mins ago was under VF. The doctor started the CPR followed by intubation, Central Venous Catheterization, 2 defibrillation, then infused adrenalin. Then, they got back the sinus rythm on the monitor, but they did not get a pulse.
Immediately, they did an Echo and found that patient had EMD. Further CPR and adrenalin ends with a fixated pupils. Patient was declared dead.
Frankly speaking, i’ve been in such situation many times. Perhaps this particular one leave an impression on me because i did not foresee the outcome and realizing that I was the last person he had spoken to before he blew his last breath.
It must has been very grimm to die alone. It must be very sad that u did not get to say what u wish to tell before u go. It must be regretful that the last person u spoken to at ur last moment is just a random med student.
I hope he would rest in peace.
Sometimes it struck me that doctors are powerless at times. Life is more about chances. U’re done anyway when ur time is up.

Lemony Snicket is my lawyer

It’s been a long long since i update my personal blog. I even left my previous blog about my travelling half written. Anyway, a lot of things happened since then.. such as:
(1) my fridge broke down
(2) my interview with CPS (commission of Public Services)
(3) Graduation Planning
(4) election of RMSA
(5) death of Mr.Stuart
(6) incident of my bathroom sink

My Fridge meets its maker :p
Let’s get the story going… Somewhere in Feb, i decided to clean-up the fridge since the freezer got too much frost on it that its door can’t be properly closed. It turns out like the chinese saying:不整尤是好,一整就没得救 (it’s okay until someone touches it). The fridge won’t turn cold anymore. I called up the service centre and asked how much it cost to repair it. They say sending a guy over will cost 300rub, while fixing it may cost 2000 rub. I got this 2nd fridge somewhere for a good 3500 rub. Served me a good 1 and a half year. Anyway, recent discussionn with roomate is concluded that we are not getting a fridge anytime soon. For one, i will only be here for another 3-4 month, whereelse he is now in an economic slump. However, problem with the fridge does give me a bit of headache since it is really inconvenient without one. My cooking habit has to change a lot since my choice of food is limited to stuff that can be kept outside the fridge. I just hope my gastritis will not flare.
Interview with the Commission of Public Services
This year, as new ruling of CPS, the interview for medical officer of public sector of Malaysia will take place both locally and overseas. Thus, there are officers being sent over to Moscow, Egypt, Jordan, England, Ireland etc to interview the graduating students. My interview was on 12th March 2008.  (see below)
As you can see, my interview is on the 1st day of the schedule of interview sessions in Russia. I’m not too worried about the interview, since there is nothing much i could do to prepare for it. Rumour has it that there will be questions on general knowledge about Malaysia, as well as medical orientated questions. Anyhow, some said that the interview might be conducted in Malay Language to gauge our language command for doctor-patient interactions. I’ve always done well in Malay Language in the past, but my skill seems to became rather rusty after 6 years being in Moscow. In fact, to certain extend, I would be more comfortable answering question in Russian Language than in Malay..haha
Anyway, Interview was a breeze. The general question are really “general” and the medical question can be answer in English, which i find no difficulty in doing. So, i’m pleased to announce that i passed my interview and “provisionally” appointed as Medical Officers.(* provisionall, because i need to first graduate from medical school :p)
As side notes, most of the students here passed the interview. So, it is nothing to be too happy about. Rather, to fail would be  something really daunting.
I promised to call home to tell my family about the outcome, but the phone was always engaged whenever i tried to call home. Perhaps i’ll just send an SMS later on.
 Graduation PlanningIf there is one thing that i’ve been keeping mself busy with, it would be getting involved in graduation planning. The Malaysian students has gather since last year to discuss about the matter, because this would be the first graduation where a large number of Malaysian Students are graduating. In fact, we are the majority among the graduating foreign students this year. (67 out of 130 )
The major concern about the event is that, it has been a lil’ disorganised last year, making it somewhat a mess during the ceremony. Many parents will be flying over from Malaysia this year to attend the convocation, so we felt the need to takes things into our own hands.
The planning was going very well until we ran into a 3rd party which interfere with our work. The 3rd party told the students that they are “assigned” to take care of the graduation ceremony and they asked us to pay them 150 USD/person. When asked, they have no apparent plans on anything and fail to present to us with the details. The funniest part is, when we plan out the event, our budget per person is around 150-180USD per person, which covers from venue, robes down to photography. At the same time, this particular 3rd party has a reputation being “profit-seeking”, yet lack the ability to properly handle tasks at hand. Thus, we decided to clarify the matter by asking the official sponsor about the matter. Funnily enough, nobody has actually be “assigned” to take care of the said event, and no one is asking for money from the students. The best part of the story is that the 3rd party dare to deny all his earlier claims and shove the responsibilities to others. Nontheless, we got the students’ support as well as the official support from embassy and major sponsor to run the event. We also clearly state that if the 3rd party were to be running the event, we wont join it. Afterall, it is pointless to attend a ceremony which u knew will be another screw-up.
Students Association Election
Follow the heat of general election of Malaysian Government, came the wave of election heat here in RSMU…hahaha. I’m  almost amazed with development of this election. I was once the pioneering batch that form the students body so i have seen the ups and downs of it (sadly, most of them are downs).
almost 2 years ago, we passed on our responsibility as student bodies to a new team. After 2.5 year holding on to the post of secretary in MMSA (Moscow Malaysian Students Association), we finally decided that it is time to pass on the job to a new team. Across the 2.5 year, the malaysian student population in Moscow grew from 400+ students to a whooping 1500+, a crowd too big that we found it hard to reach out to. At the same time, throughout that period of time, we’ve also seen many complains about the efficiency of our board in handling many of the students problems. Many problems are left unsolved, while the board member overworked ourselves every single time when we organise students events. But perhaps the reason of having many students welfare problem unsolved was the lack of real power in the student body. And eventually, lead to the distrust of students in our ability.
The previous has been a long-awaited one, where students look forward for a change. Personally, the board also think it is long over-due, because we are all tired of the responsibility proved to be too big to be carrying for that long.  During the 1st election, I’ve seen much of the power struggle among the new candidates and their supporters, each with their own agendas. We’ve seen people past getting exploited, rumours surfacing, secret being unearthed. It even progressed into a politic-like campaining and candidates joining force to run for various posts. I’ve also witnessed the reluctance some candidate to be involved, so much, as to i practically forced some to obey to the voices of the students… and with all that, a new team was borned.
2 year passed, while the new team come into power. I was the only remaining advisor for the current team. But, I have long forsaken this responsibility because i’m utterly dissapointed to find the degradation of leadership in the new team. I took no initiative to correct them. Throughout the period of the 2 year, i’ve heard numerous stories of the new team, but never had any of the team member came to consult me directly. So, i assumed they could handle things on their own. Afterall, who out wanna advise from an old-fart like myself :p
With the recent incident about the graduation planning, i came to realise that the new team not only fail to function properly. It had degraded to the extend of misuse of power. It appears that what i’ve heard was just the tip of the iceberg. And during the last 3 weeks, many incidents has been exposed. And to our delight, some of the better ones in the student body finally annouced the election of student association (RMSA) will be held soon. And to stop certain people to continue to misuse their power, the student body has also voted to suspend all resposibility of its president.
Following this were a series of event which make the election a hype. We’ve seen ppl exposing more incidents of power-misuse, complete with evedents. We’ve seen new candidates campaining for their posts. We’ve seen the darkside lurk among us. We’ve seen brave ppl speaking up.
Mr.Stuart Meets its Maker too
Last week, while me and my roomate were studying late, he went out to the toilet. To his surprised, he saw a big rat and with the overflow of adrenaline, he slam the toilet door and sqeeze the rat to death. It has been a long time since i saw any rat around my block. Not after i installed the metalic fencing blocking all entrance/exit from the pipeline compartment behind the toilet. I have no idea how the fella got in, but now that he is dead, we’ve got to get rid of the body. So i bagged him with a black plastic bag and sent him down the rubbish shaft, where i assumed he would rest in peace forever. (But I doubt that there would be peace, since he is brutally squashed, plus we didn;t provide him with a proper funeral, nor that he has any coffin to be rolling-in..muahahaha)
Bathroom Sink: The Downfall
I was rather free this weekend, so i decided that my block needs some cleaning up. It has been a while since anyone had even lift-up their finger to clean up the place. (Even the spot where Mr.Stuart died hasn’t been cleaned!). So I started off cleaning the cooking area, washing the floor, the toilet. Then, i asked my roomate to help take out the trash and also to destroy the ever-growing colony of cockroaches. Then i cleaned the bathroom and wash the moss off its wall. That is when I tripped and landed on the bathroom sink. The sink came-off the wall and I ended up with some bruises. It cost me 600rub and 4 bottle of beer to get someone to fix it up the next day.
Happy Easter
Today, it is Easter day. I would like to take the opportunity to greet everyone Happy Easter Day. Yesterday, the Malaysian Fellowship had a joint bible study session with Mrs.Terri Young (From United States). I enjoyed the stories about King Solomon. I’ve also got an Easter Egg made by the Fellowship group. Yay!

Berlin


26th Jan 2008 was the day of my last paper for the semester and 2 days later i’m off to Berlin with nothing planned out in details … i practically just pack my clothes, grabbed my flight ticket and got on the plane :p
So happens that my fren Kristof wanted to go on a trip but had no particular plan in mind, so i had him traveling with me. (despite his not too conventional name, he is a fellow Malaysia)…
Day 01 
around 5am on 28th Jan, we took a taxi to the airport. The flight is bloody too early and i end up packing my stuff+do som last minute “research” the nite before. There is a bit of a fright moment at the airport when i forgot about my pouch (which had all my important document+money+cards inside) while collecting the scanned items, only to realise it when the custom officers asked for my documents. Luckily no big scene is made, i kept my cool and explained myself, then had the Russian custom officer ascorting me to locate my “forgotten” pouch :p
The 3 hour flight is a comfortable one (especially if i were to compare it with the later flights) and we arrived safely in Berlin, Germany. The weather is not bad as compared to Moscow. We took the S-Bahn and headed for EastSeven Hostel. Upon arriving, we bumped into Olivia and her friends. Apparently, they are leaving for UK on the same day. Anyway, Eastseven is a Orangey hostel which give a feeling of being at home.
